While Medicare has covered home-based care in various forms for years, the COVID-19 pandemic significantly altered the Centers for Medicare & Medicaid Services’ (CMS’s) thinking on the types of care that could be available to beneficiaries in their personal residences. Though still in their infancy, “hospital at home” and “SNF at home” programs have received considerable attention from CMS, ACOs, hospitals, post-acute care providers, and other stakeholders exploring efficient ways to deliver advanced care. With the coordinated use of technology and multiple care delivery models, “at-home” programs are set to take center stage in the coming years.
